Our wide range of services allows us to customize our care based on individual needs. We develop a plan of care that is appropriate, effective and meaningful for each client by assessing the client’s physical, financial, social and psychological needs.
 
Once the evaluation is complete, the doctor will prescribe treatment and our care team members will create the care plan, which will be updated periodically when the client’s condition changes.

Our compassionate, trustworthy and certified Home Health Aids and Personal Care Attendants are available to assist with dressing, eating, grooming, medication reminders and light housekeeping. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. They work hand in hand with the client’s nurses, physician, and therapists to ensure that each patient’s care plan is carried out according to the wishes of the patient, their family and the patient’s physician. Our personal home care team is here to give you peace of mind, knowing that your loved one is in good hands.

Quality CareGap Services provides highly skilled, compassionate nurses on a full, part-time or per visit basis, who work closely with the client, their physician, family members and care providers to maintain the client’s health, and prevent further illness and injury. Our nurses conduct patient and environment assessments, and coordinate care with all team members. They provide treatment and educate patients and other caregivers on the disease or injury process, treatment plan, medications, diet and nutrition, foot care, catheter and wound care.
Companion Care provides emotional support and companionship for seniors who are generally healthy and want to remain independent in their home. It can also include a range of non-medical services, including basic cooking, cleaning and support that help make a senior’s life more manageable.
Our caring, professional staff offers you or your loved one customized assistance and hands-on care, twenty-four hours a day. We provide a full spectrum of private care in the client’s private home, assisted or long term care facilities, rehab centers and hospitals. Our staff will also assist with the personal care services needed to maintain independence in the home, including meal preparation, personal grooming, laundry, light housekeeping, medication reminders, companionship and caring for bed-bound clients. When one-on-one care is needed, we will be there for you, whether it’s an hour a day or if you need to reach our on-call nurse after hours.
We understand the challenges a family faces when a loved one is in the hospital, particularly if that individual needs constant observation and care. You want to be there every minute, but it’s just not possible. That’s why Quality CareGap Services offers hospital sitter services. Our professional and trustworthy caregivers are available to sit with you or your loved one in hospitals/medical facilities, nursing homes, assisted living facilities, and residences for as long as necessary. Our staff will help the client remain calm and safe by supporting their needs as they cope with limitations such as poor eyesight, hearing loss or cognitive decline. We will assist with comfort, companionship and personal care, encourage feedings and proper hydration, take notes during medical staff visits and relay that information to the family, as well as keeping the family informed of the clients status and alerting them should a problem arise.
Our skilled clinicians are experienced in caring for and managing a variety of wound conditions in order to speed recovery, increase patient independence and comfort, and reduce the incidence of wound recurrence, complications and infection. We provide care for common wound conditions, including pressure ulcers, vascular and diabetic ulcers, surgical wounds, fistulas, and skin breakdown due to incontinence.
